Steps for Enrollment
First Party Trust
The trust is funded with the Beneficiary’s own assets.
Enrollment Steps
- Review The Full Life Center Pooled Special Needs Trust Agreement with your attorney, advocate and family members.
- Complete and submit the following:
- Joinder Agreement. Please note the original must be signed by the Grantor in front of a notary.
- Enrollment Checklist
- Attorney Acknowledgement
- Signed Fee Schedule
- The Beneficiary Information Form
- The Grantor Objectives and Beneficiary Profile Form
- Duties & Responsibilities of Beneficiary Advocate
- Review Important Documents below and submit copies of all applicable documents.
- Funding Check: Make payable to FLC Pooled Special Needs Trust F/B/O (Name of Beneficiary).
Third Party Trust
The trust is funded by someone other than the Beneficiary.
Enrollment Steps
- Review The Full Life Center Discretionary Trust Agreement with your attorney, advocate and family members.
- Complete and submit the following:
- Joinder Agreement. Please note the original must be signed by the Grantor in front of a notary.
- Enrollment Checklist
- Attorney Acknowledgement
- Signed Fee Schedule
- The Beneficiary Information Form
- The Grantor Objectives and Beneficiary Profile Form
- Duties & Responsibilities of Beneficiary Advocate
- Review Important Documents below and submit copies of all applicable documents.
- The account does NOT have to be funded immediately; however, if it is funded by deferred contributions, please provide copies of the legal documents authorizing future transfer of assets to the trust sub-account. These can include: Last Will & Testament, Revocable Living Trust and/or Life Insurance Policies.
- Make checks payable to FLC Discretionary Trust F/B/O (Name of Beneficiary).
Please Note: No initial deposit is required.
Important Documents
Please submit copies of the following, if applicable:
- Beneficiary’s State Issued Identification Card
- Beneficiary’s Social Security Card
- Documentation of Beneficiary’s disability from Social Security or Physician’s Statement
- Supplemental Security Income (SSI) Benefits Verification Letter
- Medicaid Eligibility Letter or Medicaid Card
- Letter of Approval for Section 8 Housing
- Copy of Recent Bank Statement Showing Benefits Received
- Pre-paid Funeral Contract
- Letters of Authority: Guardianship, Conservatorship, Power of Attorney
- Other Relevant Court Documents
