Steps to Establish a Guardianship
- A referral for guardianship is made by completing a Request for Guardian Services form. Referral forms can be completed by nursing homes, the Court or family members.
- The Request for Guardian Services form and a copy of a recent Expert Evaluation is submitted via email to the Guardianship Program Coordinator at The Full Life Center. The Expert Evaluation must be completed and signed by a physician or licensed clinical psychologist.
- The Coordinator will assign a qualified Volunteer or family member to act as guardian and this will initiate the guardianship legal process.
- Appropriate family members will be notified and asked to sign a Waiver of Notice & Consent.
- The documents will be filed with the Tuscarawas County Probate Court and a hearing date will be set with official notices going to the ward about the guardianship proceedings.
- At the hearing, the Judge will listen to evidence presented and make a ruling concerning the guardianship.

How to Become a Volunteer
Thank you for your interest in our Volunteer Guardianship Program! We are looking for eager individuals who are:
- Willing to commit at least one year of your time
- Able to effectively communicate orally and in writing
- Willing to participate in an in-depth training program
- Able to pass FBI & BCI criminal background checks
- Over the age of 18

- Review the Guardian Program Qualifications.
- Complete and submit via email the Guardian Program Application to our Guardianship Program Coordinator.
